I don't think it's his brain, I believe it's a large skin sore on the side of his head.

I've heard that in some countries they use maggots in medicine, to have them eat infected tissue out of a wound or lesion.

Does the video involve a man or animal?. I'm done with my fix of human beheadings, decapitations & road accidents. I used to act as if they would not affect me, but I still remember many of those images/videos vividly & I'm not keen to put some more undeletable megabytes into my head....so I'll pass, unless the video involves sending mosquitoes to meet seven virgins in heaven.
