If I was the artist I would have surreptitiously tattooed nipples onto his back-boobs.

Sadly, I can't find it now, but a few years ago, a friend forwarded me a blog (message board?) entry in which a guy showed off his new tattoo, which read as follows: "You bleed to know that your alive." The words were decorated with little drips of blood.
